To glue the foam back in place, could try ShooGoo (I think, is how it is written). It has a great grip and remains flexible after drying. I Styrofoam together and works well.
Please note if you try, you might want a bit of glue to ensure DAB on a corner of the foam at first, not that they react in the wrong direction.
You can not. I recommend padded mattress covered with a vinyl zippered door, because they are cheap and are exchanged, while the mattress is not. The RV-vill provisions relating to aid protection against dust, dirt and drops bran (not stabbed or cut into slices) involuntarily.
